Directions to our Clinic Map

From Madison/Sun Prairie/Columbus area:  

1. Follow 151 North to Beaver Dam
2. Take Exit 132/ Hwy 33 exit 
3. Take a left onto Park Avenue 
4. Follow Park Avenue-staying in the left lane 
5. Take a left at University Avenue (this will be the second stop light) 
6. Follow University Avenue, you will see the Beaver Dam Community Hospital on your left. Park in the hospital parking lot. 
7. Enter in the main hospital doors. Take a left past the grand piano in the lobby. We are located on the first floor of the Vita Park Clinics, across from Marshland Pharmacy

From Fond du Lac/Waupun Area:

1. Follow 151 South to Beaver Dam 
2. Take Exit 132/ Hwy 33 exit 
3. Take a right onto Park Avenue 
4. Follow Park Avenue-staying in the left lane 
5. Take a left at University Avenue (this will be the second stop light) 
6. Follow University Avenue, you will see the Beaver Dam Community Hospital on your left. Park in the hospital parking lot.

From Mayville/Horicon Area: 

1. Follow WI-28 from Mayville into Horicon 
2. Turn Right onto HWY 33 
3. Follow Hwy 33 through Horicon to Beaver Dam 
4. Hwy 33 will turn into Park Avenue in Beaver Dam 
5. Follow Park Avenue-staying in the left lane 
6. Take a left at University Avenue (this will be the second stop light) 
7. Follow University Avenue, you will see the Beaver Dam Community Hospital on your left. Park in the hospital parking lot. 
8. Enter in the main hospital doors. Take a left past the grand piano in the lobby. We are located on the first floor of the Vita Park Clinics, across from Marshland Pharmacy
