In todays economy we understand that finances are rough on everyone. For that reason we accept and are preferred providers for many different insurances. We also offer financing and payment options for those who qualify. We want to work together to provide exceptional eyecare to everyone.

  Photo Id / Insurance Cards

The Federal Government now requires that we identify each patient that comes to our office for an appointment or to purchase anything from our optical department. This law is called the “Red Flag Rules”, and in order to comply we must require that each person present a photo ID of themselves with correct current address. If your address on your photo ID is incorrect you must present a utility bill listing your current address along with your photo ID. If the patient is a minor we will ask that a Parent/Guardian present a photo of themselves for identification for them. The only exception to this law is for persons residing in an assisted living/permanent care facility or Correctional institution who have no access to a photo ID. We thank you in advance for your cooperation in helping us comply with the Federal Governments requirements.

 

We also ask that upon arrival you present your current insurance card. This will help us file claims within a timely manor. If you do not present a card at check-in you will have 30 days to supply us with your insurance card. After 30 days we will still submit a claim to your insurance, but if any balance is not paid by insurance due to timely filing limitations that balance will be owed by the patient. We will accept an insurance card by fax to help expedite claims in situations such as this. You may also call your insurance company to check if we are preferred providers.

  Co pays/Patient Payments

If your insurance requires a co pay we will ask for this to be paid at the time of service. Any outstanding balances on your account will also be collected at that time unless a payment arrangement has been set up with our office. In addition to co pays, payment on any glasses or contacts will be due on the date they are ordered. If a contact lens fit & teach is required these charges are extra and will be due at the time of service as well. We will submit a claim for the insurances listed below; however we can not guarantee payment by your insurance. You are responsible for any co pays, coinsurances, deductible amounts and/or non covered amounts by your insurance.  Insurance payment is based upon your individual benefits; you will need to contact your insurance company for further details.

Our Office does not provide services for Badger Care UNITY and Badger Care UNITED HEALTH CARE.

 

Please be aware that there may be co-pays for your office visit or for purchase of glasses that will be collected at the time of service.

 

We are providers for Badger Care Core Plan, however this plan does not have a vision benefit, and you can only be seen for a medical reason. This plan does not cover eyeglasses.
